Summary / Zusammenfassung Human immunodeficiency virus type 1 (HIV-1) is the cause of a devastating pandemic that affects ~35.3 million people in 2013. Its life cycle allows HIV-1 life-long persistence and one crucial step in establishing a viral reservoir is the capacity of HIV-1 to integrate its genome into the human genome. Combinations of antiretroviral drugs are highly effective in dramatically reducing mortality by inhibiting viral replication but they cannot eliminate the viral reservoir. Understanding how HIV-1 can persist is crucial to the development of strategies to eliminate this viral reservoir.
Our project has the ambitious goal of defining host and viral factors that play a role in the pathomechanism of HIV-1 latency. To achieve this goal, we will take advantage of the Swiss HIV Cohort Study (SHCS), a 1988 founded, nation-wide cohort including more than 50% of all registered HIV-1 infected individuals in Switzerland. Anonymized clinical data and longitudinal cell and plasma samples of more than 18,000 HIV-1 infected individuals are archived. We will study the group of approximately 1,600 individuals who received antiretroviral therapy (ART) for at least 5 years and whose virus was suppressed in plasma to below the limit of detection in this time period. Decay rates of the viral reservoir as well as host and viral genomes of those individuals will be generated using state-of-the-art methodologies. These complex data sets in the context of clinical parameters such as CD4+ T cell recovery during ART, drug regimens, co-infections, etc. will be used to define host and viral factors having an impact on the pathomechanism of HIV-1 latency. Models will be established to predict treatment responses and to identify the group of HIV-1 infected individuals who might have the best chances of being cured when effective reservoir depleting drugs become available.
