| Summary / Zusammenfassung | Background Conditions referred to as “metabolic” or potentially related to premature aging (cardiovascular and cerebrovascular disease, osteoporosis, neurocognitive dysfunction, diabetes mellitus, lipodystrophy, endocrinologic disorders) are increasingly important determinants of the long term health of HIV-infected individuals. Given the limitations of the current SHCS data collection format, the SHCS Scientific Board has recognized the need for a major, coordinated effort to increase the incisiveness and productivity of metabolic and aging-related research within the SHCS. The SHCS has already successfully established and is productively conducting 2 core projects (Resistance and Genetics). Study Aims 1) To collect fasting stored urine and plasma samples in 2000 SHCS participants every 2 years, to allow biomarker protocols requiring fasting samples (biomarkers involved in glucose metabolism, insulin resistance etc.) and discovery protocols based on metabolomics and other emerging –omics methods. 2) To establish a nation-wide, longitudinal “core metabolic and aging (M+A) cohort” of 1000 SHCS participants within the existing SHCS infrastructure, recruited from among the 2000 participants who provide fasting urine and plasma samples. 3) This is not a research protocol but a core infrastructure project which aims to enable, stimulate, and coordinate research on M+A topics in SHCS participants through SHCS SB protocol submissions by all SHCS-affiliated researchers 4) To generate and contribute new levels of data to the field of metabolic and aging-related complications 5) To make project submissions more competitive for external funding, including Swiss National Research Foundation (SNF) applications, SHCS-international, and SHCS-industry collaborations Study Design Infrastructure project: Collection and storage of fasting plasma and urine samples in 2000 SHCS participants every 2 years. Establishment of a core M+A cohort of 1000 of these 2000 SHCS participants within that are longitudinally followed every 2 years and in whom DXA, coronary imaging, neurocognitive testing and comprehensive M+A genotyping will be done. Only inclusion criteria: age >45 and informed consent provided (no ART criteria etc.). Governance and review within existing infrastructure (SHCS Scientific Board): research protocols based on M+A cohort participants will be submitted as Full Proposals to SHCS SB in the same fashion as other SHCS full proposals. |
